We want you to feel comfortable at Retina Consultants. Here are some important things to know in preparation for your first visit:

  • Please plan on 1 to 1.5 hours for your first visit.
  • Bring sunglasses (rain or shine) because your eyes will be dilated for the first visit. If possible, arrange for someone to drive you home.
  • If your insurance requires co-payments, we accept cash, check, credit or debit cards.

Please review our financial policy, then print, complete and bring the two patient forms to your first visit:

Other items you’ll need for your first visit:

  • Your current medical insurance card(s), as well as billing information.
  • Picture ID such as a drivers license.
  • A list of all medications you are currently taking.
  • If your insurance requires authorization for you to see a specialist, please bring an insurance referral which is separate from your insurance card. This is typically obtained from your insurance company or your primary care physician. If you have any questions or concerns about insurance or billing, please contact our patient accounts department at 425-791-3017. Our patient accounts department office hours are Monday – Friday, 8:00am – 4:30pm.

Again, please bring all requested items with you to your appointment, this will help expedite the check-in process. Don’t hesitate to call if you have any questions. We are here to help!
