PATIENTS WHO ARE ILL (fever, chills, cough, sore throat, shortness of breath or difficulty breathing) ARE NOT ALLOWED INTO OUR OFFICES.
Regardless of your health, ALL PATIENTS MUST WEAR A PROTECTIVE FACE COVERING UPON ARRIVING TO OUR OFFICE. Please bring your own mask as we do not have masks to distribute.
AFTER CHECKING IN FOR AN APPOINTMENT, PATIENTS MAY BE ASKED TO WAIT IN THEIR CAR OR OUTSIDE OF OUR CLINIC SUITE. We will call a patient’s cell phone when it is time to come into the clinic for their appointment.
NO FAMILY MEMBER/FRIEND ALLOWED TO ACCOMPANY A PATIENT (unless medically necessary).
IF YOU HAVE TRAVELED OUTSIDE OF THE AREA IN THE LAST TWO WEEKS, HAVE HAD KNOWN EXPOSURE TO COVID 19, OR TESTED POSITIVE TO COVID 19, YOU MUST CALL THE OFFICE PRIOR TO YOUR APPOINTMENT AND NOTIFY STAFF. Travel and exposure may affect your ability to be seen by our medical staff.
