How To Refer A Patient

To refer a patient to Retina Consultants of Seattle please call our office with the patient's information. To allow us to provide the best care for your patients please fax a letter describing the reason for the referral and a summary of any diagnostic tests that have been performed.

To assist in making your patients care with us a smooth transition, we have referral request forms available, as well as brochures, new patient packets, and business cards. If you are interested in receiving any of these items, please call either office, and we will happily mail them to you the same day.
